I really like the Liverpool Care Pathway, and lots of hospitals here in the UK have adapted it.

Overly long and repetitive article, but serious food for thought anyway. The sheer indignity of forced extension of life is one of the serious down-sides of medical advances. As the article says, it's important to know when to stop and even more important to remember this when it matters.
