Add to it throughout your work day when tasks trigger that "oh, yeah, now I do this too" and "I designed this"?

Ug, that's always a difficult one. I do always use those reviews to toot my own horn though, you need to be sure that someone is getting your accomplishments down on paper.

I always start out with the ubiquitous 'The end of 2008 marked the end of a difficult and challenging year which myself and my team persevered through.'

… then sneak in how you managed to save the world and make everyone tea.
