I would not call that type of facial expression "agony" so much as "intense concentration". IMHO there is very little actual agony involved.

stilicho, I've heard that story with regard to a couple of films. Both The Blue Lagoon and Great Balls of Fire spring immediately to mind, and I'm sure there are a few others.

IIRC, it was Brooke Shields toe in Endless Love. I checked the trivia thingie on IMDB but it doesn't say anything about it.
