It's funny - he looks young and old at the same time. He had hip replacement surgery, a couple of cancers, and maybe a little plastic surgery too?

Well, Amanda LePore is a special case. She was not born a woman (and may "technically" still be male--I don't remember) and her exaggerated features are meant to be a type of artistic statement, IIRC.

Jocelyn, on the other hand, is seriously mentally ill--hacking her face away in an attempt to keep her lion-loving husband. She thinks she's purdy.
